Home / Admissions
Moving in, step by step.
Whether you're planning ahead or calling from a hospital waiting room, we'll guide you through each step and handle as much of the paperwork as we can.
Call and talk with a nurse
Tell us about your loved one. We'll answer questions, explain costs, and tell you honestly whether we're a good fit.
Tour the home
Visit in person or by video. Meet the owner and caregivers, see the open room, and stay for a meal if you like.
Nurse assessment
We meet your loved one, review health history and medications, and gather the state-required forms with you.
Move-in day
We plan the move together, set up the room with familiar things, and check in with you daily during the first weeks.
Paperwork
The forms California requires.
Every licensed RCFE in California uses the same state forms. We'll give you a complete packet and help fill it out.
- Physician's Report for Residential Care Facilities for the Elderly (LIC 602A), completed by your doctor
- Tuberculosis screening or test results
- Pre-placement appraisal and resident information
- Current medication list and prescriptions
- Admission agreement explaining services, rates and policies
- Resident rights and personal property record
- Copies of insurance cards, ID and any advance directives or POLST
Costs and payment
One rate, in writing, up front.
Our monthly rate is all-inclusive and based on the level of care your parent needs. No surprise fees for laundry, medication management or activities. Call for current rates and availability.
Ways families pay
- Private pay
- Long-term care insurance (we help with the claim paperwork)
- VA Aid and Attendance benefits for eligible veterans and surviving spouses
- Proceeds from a home sale, reverse mortgage or family contributions
Medicare doesn't pay for room and board in assisted living, but it may cover hospice, home health visits and doctor care while your parent lives here.
Before you choose
A checklist for any care home you visit.
We'd rather you choose well than choose us. Use this list on every tour, including ours.
- Look up the home's license and inspection history on the CDSS Care Facility Search
- Ask who owns the home and how often they're there
- Ask who works overnight and how many caregivers are on each shift
- Watch or share a meal
- Ask what happens if care needs increase
- Get the full monthly cost and move-out terms in writing
- Visit twice, at different times of day
