Home / Care & services
Personal care, planned by a nurse and given by people who know your parent.
From a little help each morning to comfort care at the end of life, we adjust the care plan as needs change, so your parent can stay in one home.
Care that fits where your parent is today.
Each resident's plan starts with a nurse assessment and is reviewed regularly and whenever something changes.
Assisted living
For older adults who need daily help but want to keep their independence.
Support with bathing, dressing, grooming and medications in a warm home with company around the table. Residents set their own pace; we fill in what's needed.
- Help with daily routines
- Medication management
- Meals, housekeeping, laundry
Memory support
For residents living with mild to moderate dementia or memory loss.
Small homes can be easier for people with dementia: fewer people, less noise, and the same faces each day. We use steady routines, gentle redirection and meaningful activity.
- Consistent daily rhythm
- Secure, calm environment
- Caregivers trained in dementia care
Hospice and comfort care
For residents who want to remain at home through the end of life.
We partner with the hospice agency of your choice so your loved one can receive comfort care in their own room, with family welcome at any hour.
- Coordination with hospice nurses
- Comfort-focused personal care
- Open visiting for family
Respite and short stays
For family caregivers who need a break, or after a hospital stay.
When a room is available, short stays let a family caregiver travel or rest, or give your parent a safe place to recover before going home.
- Stays of a few weeks or longer
- Same care and meals as long-term residents
- A good way to try the home first
Services
What's included every day.
Nurse-led care planning
Every resident has a written care plan built from a nurse assessment and reviewed as needs change, not only once a year.
Medication management
Medications stored securely, given on schedule, logged on every pass, and reconciled after every doctor visit or hospital stay.
Help with daily routines
Bathing, dressing, grooming, toileting and continence care, given with patience and in private.
Mobility and fall prevention
Steady help with walking and transfers, a single-level-friendly layout, grab bars, and a watchful eye on fall risk.
Home-cooked meals
Three meals a day plus snacks, cooked in our kitchen, with care for diabetic, soft, low-sodium and cultural diets.
Memory support
Calm routines, gentle redirection and familiar faces for residents living with mild to moderate memory loss.
Hospice and comfort care
We work alongside licensed hospice agencies so residents can stay in the home they know, with the people they know.
Care coordination
We schedule appointments, arrange transport, talk with doctors and home health, and keep families informed.
Housekeeping and laundry
Rooms tidied daily, linens and personal laundry washed on site, clothes returned folded.
Activities and company
Music, gardening, crafts, games and conversation, planned around what each resident enjoys.
Appointment help
We schedule doctor visits and help arrange transportation, with notes shared back to family.
Health monitoring
Weights, vital signs and changes in mood or appetite tracked and shared with doctors when needed.
Working with your doctors
We coordinate. Your parent keeps their own care team.
Residents keep their physicians, specialists and pharmacy. We make the calls, keep the records, and make sure new orders are followed.
- Primary care doctors, specialists and telehealth visits
- Licensed home health agencies for nursing visits, therapy and wound care
- Hospice agencies of the family's choosing
- Pharmacies with blister-pack or delivery service
- Mobile labs, X-ray, podiatry and dental services that come to the home
An RCFE is a non-medical care setting under California law. Skilled nursing services are provided in the home by licensed outside agencies, which we coordinate. If needs ever go beyond what we can safely provide, we'll help you plan the next step.
Not sure what level of care fits?
Call and talk with a nurse owner. We'll listen, ask a few questions, and give you an honest answer.
